To keep things as easy as possible, we’ve streamlined the process to reduce any disruption to your care and services when changing homecare package providers.

Local care manager
Homecare My Way is the first provider to offer self-management with a care manager who is local to you.
Unlike most self-management providers who only offer phone support, we provide personal, in-person support with unlimited face-to-face visits whenever you need it.

4 Easy steps to switch providers!
1 Assessing the switch
Speak with us to determine if switching providers is right for you. We’ll discuss your reasons and provide a comparison to show how managing your homecare package with us differs from your current provider.
2 Initiating the switch
If you decide to proceed, we will contact My Aged Care together to reactivate your referral code and arrange a home visit with your new Care Manager to finalise your care plan.
3 Notifying your current provider
Inform your current provider of your decision to move your homecare package and agree on an exit date. Once confirmed, let us know and you can select your care and services to begin with Homecare My Way on the agreed date.
4 Transitioning funds
Your current provider has 70 days post-exit date to transfer any unspent funds to Homecare My Way. During your first 70 days with us, you can utilise care and services up to your monthly funding amount.
Once your unspent funds are transfered, you’ll be able to access this for your care and services. Your care manager will guide you through this process so you can manage your budget effectively.
Choose the right care management model when switching to Homecare My Way
At Homecare My Way, we offer two flexible care management services: Self Management and Self Management Plus.
Self Management
If you value independence and want direct control over your care, our Self Management model gives you just that. You’ll be able to choose, hire and coordinate your own team of service providers according to your care plan. Once registered with us, we handle all the necessary compliance checks to ensure quality care.
With Self Management, you’re in charge of day-to-day scheduling and adjustments, while we’re always here to provide support and guidance as you transition smoothly to your new provider.
Self Management Plus
For those who prefer additional assistance in setting up their care, our Self Management Plus model provides extra support. We’ll handle the initial steps of finding, onboarding and conducting compliance checks for your care providers, ensuring they align with your needs. Once your team is established, you can take over the day-to-day management, with regular check-ins from us to help you stay on track and make the most of your package.
